Premier Physical Therapy is continuously accepting new patients. To make an appointment, please call (972) 608-3939. It is also requested that new patients fill out the New Patient Information Forms prior to their visit in order to expedite insurance processing. The New Patient Information Form can be filled out and submitted online or by downloading the document, printing it, filling it out, and faxing it to our office at (972) 608-3938.
You need to bring: X-rays (original films, if possible). The hospital will not mail the films. Premier Physical Therapy will keep the films in your file unless instructed to return them. Also bring any diagnostic studies (MRI, CT scans) and your insurance card and referral (if needed).
Premier Physical Therapy requires a consent form that is signed by either the patient or the patient’s legal guardian.
